Mid-Life and Mid-Face

A mid-life crisis is usually brought about by the realization that you have passed through at least the first half of your life. It can lead to introspection and can give you a chance to reflect on your life. Another thing that comes with mid-life crisis is the mid-face crisis. As we get older, we lose boney mass all over the body.

This leads to shortening of our height as we get older. The bone loss happens in our cheek bones as well. As the cheek bones shrink into the face they cannot hold up the beautiful apple cheeks from our youth. This allows descent of the soft tissues towards the lower jaw and the corner of the mouth. The tissues also becomes lax and loses its elasticity.

We tend to blame gravity for this, but the reality is that even if we lived on the moon the face would still age and the tissues would descend. This can lead to a lot of changes in the mid face including hollow cheeks, excess wrinkles, deep smile lines (nasolabial folds) and so on.

In the earlier years, all doctors emphasized the importance of reducing the parentheses (nasolabial grooves) with fillers. Something always told me that there is more to it than meets the eye. I started observing beautiful faces and realized that many beautiful women had deep nasolabial lines and it did not affect their looks. That was an ‘A-ha!’ moment for me. I began to realize that what takes away the beauty of the mid face is the imbalance between cheeks and the lower face.

Hence, I started promoting restoration of the cheek area to restore the normal anatomy and harmony of the mid face. For this I used many fillers, but found Sculptra to be the best in terms of its subtleties, slow progression and longevity. This has become a well known practice among all plastic surgeons where the emphasis is now on getting vertical or diagonal lift of the cheeks. It will also help around the mouth using Sculptra, Voluma, or Radiesse. This approach is more aesthetic than just filling in the nasolabial grooves.
