Asian eyelids are different then Caucasian eyelids primarily because of lacking an upper eyelid crease. To change that appearance the ‘Double Eyelid’ surgery has been used. There are two approaches for this surgery. One is similar to regular blepharoplasty technique involving incision in the upper eyelid at the desired height of a crease.
The crease is created with suturing the skin to the Levator muscle. The second approach uses stitches going through the skin of the eyelid without any cutting. This achieves the same result with less bruising or down time.
My approach: I like to use the open technique of removing a small amount of skin in the desired eyelid crease and then suturing carefully to close the skin with a bite into the Levator muscle. This allows for a more predictable and permanent double crease.
